 Insurance

I am considered an “out-of-network” provider which means that I do not accept health insurance of any kind and session fees will be paid directly by the client following each session. You may be eligible for out-of-network benefits through your insurance provider (often times this will cover a large percentage of the fees).

As an out-of-network provider, I have partnered with Mentaya to help you easily understand your out-of-network benefits. Use the tool below to see if you qualify for reimbursement for my services once you’ve met your deductible.

If you feel more comfortable calling your insurance company please scroll down for a list of questions to ask to understand your reimbursement eligibility.

Do I have out-of-network benefits for mental health coverage?

If so, what percentage is covered?

What is the deductible and how much of it have I met?

What is the co-pay for a session if I see an out-of-network provider?

What is my coverage for an out-of-network provider who is licensed in zip code 10028?

CPT Code: 90837 for a 60-minute individual therapy session

CPT Code: 96130 for psychological testing (billed up to 10 hours)

CPT Code: 96132 for neuropsychological testing (ADHD testing; billed up to 10 hours)

What is the process for submitting the “superbill” and receiving reimbursement?

If you would like to submit a claim of therapy to your insurance company, I will happily provide you with a monthly “superbill” which will include all of the necessary information for your claims to be processed. This will also be your paid receipt.

“No Surprise Act”: Good Faith Estimate

(effective January 1st, 2022)

The "No Surprises" Act requires health care providers to provide current and new patients who are uninsured or self-pay with a "Good Faith Estimate" (GFE) of cost for treatment. This regulation attempts to provide patients with transparency regarding their health care costs.

Many factors make it difficult to provide an estimate given that the length of therapeutic services is dependent on the individual client and their goals for treatment. Ultimately it is the patient who may decide to stop treatment at any time.

If the patient is uninsured or using self-pay, Dr. Gluck will provide the patient with a GFE orally and in writing. The example below may help the patient have an idea of the financial expectations for starting therapy with Dr. Gluck, however this is variable based on the session fee, duration and frequency of therapy agreed upon by the patient and Dr. Gluck. The GFE is neither binding nor a guarantee that the patient and therapist will work together. The following example of a GFE is for a 12 month period:

Individual 45-50 minute session: $275

Weekly 45-50 minute session for 52 weeks: $14,300

Bi-weekly 45-50 minute session for 52 weeks: $7,150
